Services

What we provide for residents

Every resident has an Individual Service Plan (ISP) and, when needed, a Behavioral Support Plan (BSP). Our services are designed to support daily living, health, behavior, and quality of life.

Daily living & personal care
  • Bathing, grooming, dressing, toileting, and continence care
  • Assistance with walking and transfers
  • Personal hygiene and reminders
  • Support to maintain independence where possible
Medication & health support
  • Medication administration and management
  • Quarterly medication review and staff training
  • RN available 24/7 on-call to assist with health status
  • Monitoring and reporting changes to physicians
Behavioral support & safety
  • Behavioral Support Plans (BSP) on admission as needed
  • Clear interventions for challenging behaviors
  • Communication with guardian, POA, and care team
  • Ongoing review and revision of plans
Social, functional & life skills
  • Light housekeeping, laundry, and meal preparation skills
  • Encouraging communication and self-advocacy
  • Support with routines and personal interests
  • Practice of daily social skills in a group setting
Meals, activities & community
  • Daily meals and healthy snacks
  • Special diets and dining assistance as needed
  • Recreational and social activities
  • Outings to parks, museums, festivals, libraries, and events
Medical coordination & transportation
  • Scheduling and coordinating medical, dental, psychiatric visits
  • Transportation support as appropriate
  • Documentation of visits kept in the resident’s record
  • Collaboration with providers for ongoing care plans
Our approach

How we plan and monitor care

We don’t do “one-size-fits-all.” Every resident’s plan is built around their history, needs, values, and goals – with ongoing review and input from family and the care team.

Individual Service Plans (ISP)

Upon admission, we develop an Individual Service Plan for each resident. The resident’s legal guardian, family, RN, and Goldcrest Care staff are invited to participate.

Each ISP includes:

  • Medical and behavioral history
  • Current abilities and support needs
  • Daily living and social goals
  • Risk and safety considerations
  • Community inclusion and recreation
  • Long-term plans and preferences

Plans are reviewed at least every 6 months or sooner if there is a change in status. Progress can be shared with guardians and involved team members regularly.

Program goals & staff training

Our goals for every resident include:

  • Reaching their highest level of functioning
  • Maintaining rights and independence
  • Building self-esteem and self-respect
  • Encouraging hopefulness and stability
  • Supporting meaningful daily structure
  • Strengthening community integration

Staff receive training in medication administration, standard precautions, first aid and choking, fire safety, dietary needs, resident rights, and recognizing/responding to challenging behaviors. Continuing education is completed per state requirements.
